CONNECTION

Each client is assigned a dedicated Remote Care Coordinator (RCC) who monitors health data, tracks the care plan, and keeps families across Palm Beach County informed and supported.

DEPLOYMENT

A simple, intuitive tele-health kit is personalized for each client. This technology captures health data based on clinically defined thresholds for accurate, real-time home care monitoring.

OBSERVATION

Our remote monitoring center enables efficient coordination of in-home care, supporting safe transitions from hospital to home and reducing the risk of unnecessary readmission.

INTERVENTION

We use meaningful clinical data to continuously refine each client's care plan, keeping their needs, independence, and dignity at the center of every decision made.
